I believe the ringback tone is provided by your carrier. Sprint offers this service.

AT&T and Verizon both offer them for sure.

It's like $1.99 a month, and $1 for every new tone. Verizon did have some pretty specific customization options. I'm not sure about AT&T.

The best/strangest/worst part is when somebody calling you is singing the song when you answer, and then they harass you about your choice of music.
