With my Etymotic hf2 headset, a long press of the button will initiate TellMe, just as a long press of the Windows key will on the phone itself.

How exactly does it work? Is there a dedicated button to start listening? Or is it a combination of buttons to press?

I can confirm with my Nokia wh-208 headset you just long-press the tap-to-talk button and it's just like if i long-pressed the Windows button.
And since I spent my Bing rewards points to buy the full version of Hey DJ I have full voice controlled access to my entire music library through my headset.
